Application Information

Note: Payment must be made prior to your desired class date to secure your spot in the class.

There are three ways to apply for one of our classes. If you are paying by check, please follow this link to our application. Simply print the application, fill it out, and mail it to us with your check. Instructions are on the application.

If you are using American Express, Visa, Master Card or Discover to pay for the class, you can follow the link above, print out and mail or fax the application to us (instructions are on the application) or you can follow the link below to our on-line application. Fill it out, click on submit, and your application is on the way. On-line application users must phone their credit card information to us at 816-478-3677 within 1 business day of submitting the application. Payment must also be at least 2 business days prior to the scheduled class.

Students must complete the course with the class with which they began the training. If the instructor determines the student is not eligible to complete the class or if the student voluntarily decides to leave the class, refunds and/or rescheduling are not available.
